At Ottoville Local Schools, we welcome all students who live within the district's boundaries and meet state age and health requirements. For specific enrollment information, please contact your child's school. If you need to find out which school your child will attend, please use the School Finder to look up the attendance boundaries for your address.

- Parents and/or Guardians may enroll students at Ottoville Local School.
- Please contact Ottoville Elementary School at 419-453-3357 if enrolling a student grades K-5th.
- Please contact Ottoville High School at 419-453-3358 if enrolling a student grades 6th-12th .
- School Office Hours are 7:45am – 3:15pm Monday – Friday.

- Proof of Child's Age: An official birth certificate, passport, or hospital record.
- Parent/Guardian ID: A valid state ID, driver's license, or passport.
- Proof of Residency: Documents like a recent utility bill (dated within 30–60 days), lease/rental agreement, or mortgage statement.
- Immunization Records: Your child's current vaccination/shot records (e.g., DPT, Polio, MMR) or an official exemption form.
- Custody/Guardianship Papers: If applicable, file-stamped court documents proving legal custody or guardianship.
Additional paperwork (like previous school transcripts or an IEP) will be required for transfer students.

Online registration can be access via Final Forms. New parents can signup for a new account. Parents who already have a Final Forms accounts SHOULD NOT create a new account. Please log in with your existing Final Forms account and select the "New Student" button to register your upcoming student.
Instructions:
- New Parents: Select the Blue "New Account" button
- Existing Parents: Select the Black "Log In Button"
- After your child is registered, you will receive an email from FinalForms with required forms to complete.

How do I open-enroll my child?. Contact the district office for questions regarding open enrollment and transportation.
Do I need to complete Final Forms every year?
Yes. The information entered into FinalForms need to be completed every year. FinalForms should also be updated throughout the school year if your address, phone, etc… change. FinalForms information is what we use to contact parents.
What is Open Enrollment and how does it work? Many districts accept students who live outside their boundaries through Open Enrollment. Admission is based on space availability and district policy. Parents must inquire with the receiving district's enrollment office during the spring application window, and parents are typically responsible for transportation from outside the district.
